Vendor & Supporter Sign-up Form
How do I become a "vendor"?
You can become a vendor by submitting the online vendor form. Be sure to fill out the form completely and accurately. Shortly after you submit the form, you will hear from a South Huntington UFSD Wellness Fair official confirming your attendance as a wellness fair vendor. All online vendor forms must be submitted on or before Friday, March 1st. If you are not able to complete the form online and wish to complete it through traditional methods, print out the form, complete it and fax it to Ms. Raspantini at 631-812-3165 or mail it to SHUFSD, 60 Weston St., Huntington Station, NY 11746. If you are unable to print the form, please contact E. Caputo at 631-812-3151 and a form will be sent to you. There is no cost involved with becoming a vendor.
Can a "vendor" sell things at the fair?
No, as a vendor, you would not be selling items or services at the health and wellness fair. Of course you may end up with clients or customers in the end who will support your business, but at the day of the fair, there will be no money transactions.
What about Providing Samples?
Although money will not be changing hands during the actual fair, you can provide samples of your goods or services if you wish. Please let us know using the online vendor form if you wish to provide samples to the fair attendees. Some vendors in the past have provided food samples. If you do wish to also provide food samples, we ask that the food keep in the spirit of wellness. This means no candy or sugary treats. You should also be a licensed food service and provide your ingredients upon request in case of allergies. You should also make yourself familiar with our Board of Education Wellness Policy on the district's website.
Wellness Fair Attendance Policy
If you would like to be a volunteer vendor at our next Health and Wellness Fair, keep in mind that there is a limited space and not everyone who wants to be a vendor might be able to participate. Due to this, we ask that anyone who is volunteering as a vendor be willing and able to stay for the complete length of the fair. Set up time is 3:00, ready to welcome attendees promptly at 4:00. If you think you might have other responsibilities that could force you to leave the fair early, perhaps reconsider being a vendor and ask how else you can become a participant.
*A Certificate of Insurance is required from all vendors.
